​Follow these 3 easy steps to schedule your appointment​
1. Choose a provider
  1. Go to the bottom of this page under "Choose a Provider" and select the type of service(s) you need (medication management, counseling or both)
  2. Use the categories and information provided to narrow your search
  3. Click on a provider's name to view their profile page where you can learn more about them, their areas of expertise and if they are accepting new patients
After choosing a provider,  PROCEED TO STEP 2: Select an Appointment
2. Select an appointment
For medication services:
Click SKIP on the scheduling screen, as no appointments will show available. You will need to call us after registering for medication services to schedule your appointment.
PROCEED TO STEP 3: Complete registration/forms and finalize appointment

For counseling services:
  1. Click the "Schedule" button at the top of the provider's profile page. You will be directed to their Therapy Appointment registration portal. ​
  2. Click "New client of  ____ - book an appointment."
  3. Select from the dropdown options under Service and Telehealth to populate the calendar showing available appointments.
  4. Click on date and time to hold the appointment while you register (appointment will be finalized after registration is complete)

PROCEED TO STEP 3: Complete registration/forms and finalize appointment

Please allow at least 20 minutes to complete the process so you are not rushed or unable to finish.

If your session does get interrupted at any point, call us at 614.360.2600, option 1. Depending on where you are in the process, your progress may not have been lost and we may be able to get you into the patient portal to finish. 

Follow the prompts to complete each tab in the registration process. Click CONTINUE at the bottom of each screen to advance to the next section.

1. Complete:
  • Demographics (name, address, phone number, email, etc)
  • Insurance information, if applicable
  • HSA or credit card information (used for any applicable copays/fees)
  • Consents/new patient forms

2. Confirm appointment

3. Activate your patient portal (REQUIRED)
  • Click "Set up Username and Password" button on the final screen
  • Enter your birthday and create a Username and Password. 
  •  You have now activated your patient portal, which is necessary for telehealth.

HELP!
If you get stuck, or your session is interrupted before you finish, please call us at 614.360.2600, option 1, and we will give you access to your patient portal to finish the process.

Choose which counselor and medication provider you would like to see following the instructions under each category.

For medication services with Laura Davis, in addition to counseling services:
  1. Select a counselor and register/schedule for counseling service. (Follow steps under "counseling" section.)
  2. Call us at 614.360.2600, option 1, to have Ms. Davis added to your chart and an appointment scheduled with her. Please do not duplicate your chart by registering twice.

For medication services with Dr. Churchill, in addition to counseling services:
  1. Select a counselor and register/schedule for counseling service. (Follow steps under "counseling" section.)
  2. Complete the pre-scheduling screening so Dr. Churchill can be sure she is the best fit for you. We will  review your submission and contact you within 3 business days regarding next steps.
PARENTS: If your child is 18 years or older we are NOT permitted to talk to you, even to schedule/reschedule appointments, unless there is a signed Release of information form (ROI) on file.
