​Follow these 3 easy steps to schedule a counseling appointment
Therapists/psychologists do NOT prescribe medications.
If you need medications, please select a prescriber.
If you need both counseling and medications, follow instructions provided HERE
1. Choose a provider
  1. Go to the bottom of this page under "Choose a Provider" 
  2. Use the categories and information provided to narrow your search
  3. Click on a provider's name to view their profile page where you can learn more about them, their areas of expertise and if they are accepting new patients
After choosing a provider,  PROCEED TO STEP 2: Select an Appointment
2. Select an appointment
APPOINTMENT WILL NOT BE BOOKED UNTIL YOU FINISH REGISTERING AND ACTIVATE THE PATIENT PORTAL IN THE LAST STEP. 
  1. Click the "Schedule" button at the top of the provider's profile page. You will be directed to their Therapy Appointment registration portal. ​
  2. Click "New client of  ____ - book an appointment."
  3. Select from the dropdown options under WHAT (Service) and WHERE (location) to populate the calendar showing available appointments.
  4. Click on date and time to hold the appointment while you register (appointment will be finalized after registration is complete)

PROCEED TO STEP 3: Complete registration/forms and finalize appointment. APPOINTMENT IS NOT BOOKED UNTIL YOU FINISH REGISTERING AND ACTIVATE THE PATIENT PORTAL IN THE LAST STEP.
3. Complete registration/forms and finalize appointment
Please allow at least 20 minutes to complete the process so you are not rushed or unable to finish.

If your session does get interrupted at any point, call us at 614.360.2600, option 1. Depending on where you are in the process, your progress may not have been lost and we may be able to get you into the patient portal to finish. Please do NOT start over before calling us.

TIP: The registration window is a POP UP window. This means if you click outside of it at any time during the registration process it will disappear. (You may think you were "kicked out")  Try not to click outside the window to prevent it from closing. If it happens, please CALL US at 614.360.2600, option 1.

Follow the prompts to complete each tab in the registration process. Click CONTINUE at the bottom of each screen to advance to the next section.

1. Complete:
  • Demographics (name, address, phone number, email, etc)
  • Insurance information, if applicable
  • HSA or credit card information (used for any applicable copays/fees)
  • Consents/new patient forms*

2. Confirm appointment

3. Activate your patient portal (REQUIRED)
  • Click "Set up Username and Password" button on the final screen
  • Enter your birthday and create a Username and Password. 
  •  You have now activated your patient portal, which is necessary for telehealth.

​PAPERWORK
*If you SKIP the paperwork during the registration process, you MUST log in to the patient portal and complete the paperwork at least 2 days prior to your scheduled appointment. (Under Docs&Forms in your portal.)

HELP!
If you get stuck, or your session is interrupted before you finish, please call us at 614.360.2600, option 1, and we will give you access to your patient portal to finish the process.
