Scheduling your appointment online is easy!

STEP 1: Select a therapist
Click here to view a list of providers. Read about their specializations and choose the one that would be a good fit for you. STEP 2: Register and Schedule
Click "schedule" by the therapist's name on Our Provider page or at the bottom of their profile page to be taken to the registration screen. Click "register as a new patient" and complete required information. After registration is complete, CHECK YOUR EMAIL for a link from no-reply@therapyappointment.com with the link to activate your patient portal account and set up your user name and password. THIS LINK EXPIRES IN 48 HOURS, so be sure to click on it after registering so you have access. If it expires, call us at 614.360.2600, option 1, to be sent a new link. You will then schedule your appointment through the patient portal. Click here to access the patient portal login screen STEP 3: Complete new patient paperwork
The paperwork will be assigned to you in your patient portal, under Docs & Forms. The paperwork needs to be received at least 48 hours PRIOR to your appointment or your appointment will be rescheduled. |
Note: we do NOT complete disability or FMLA paperwork, write ESA letters, complete pre-operative bariatric forms nor provide ADHD testing.
