Do you provide ESA letters, Disability letters, pre-operative bariatric assessment, or other letters for accommodations for college housing or the workplace?
No, we do not. Please contact your primary care doctor if you need resources for these types of services.
Do you offer ADHD Testing?
No, we do not, but here are some other practices that do offer ADHD testing. We do, however, have Dr. Cynthia Churchill, a psychiatrist who specializes in medicating ADHD adults age 18 and over, and Carla Morris, LPCC-S, a therapist who specializes in treating ADHD.
Whom do I contact if I have difficulties filling my prescription?

Why can I as a parent not schedule appointments for my 18-year-old child?
If your child is 18 years or older, for HIPAA reasons we are NOT permitted to talk to you, even to schedule/reschedule appointments. Please ask your child to sign a Release of information form (ROI) if you need to speak with us.
Do any of your providers see patients in person or is CBH offering only virtual?
