FAQ for Current/Returning Patients
All appointments are virtual.
Patients must physically be in Ohio, even for virtual visits.
We upgraded to TherapyAppointment 2.0 on 4/28/21.
If you are a current patient, you should have received an invitation to activate your patient portal in the new system.
Your old login will NOT work in the new system. You will have to set up a a new account using the link sent to you.
If you need a new link, please call 614.360.2600, option 1, and request that the link be resent.
Need to schedule, reschedule or cancel an appointment?
You can do so using the PATIENT PORTAL, by contacting your provider directly or by calling 614.360.2600.
If you need to cancel an appointment after office hours, use the patient portal, contact your provider directly or leave a message on the main line. Messages are time/date stamped and we will use this information to determine if the cancellation was outside 48 hours.

Cancelling less than 48 hours in advance?
Contact your provider or call 614.360.2600, option 1.
Appointments cancelled less than 48 hours in advance are considered late cancellations and are subject to a late cancellation fee, at the provider's discretion. These fees are not billable to insurance and are fully your responsibility.
$0 for first late cancel/no show in calendar year
$65 for 2nd late cancel/no show in calendar year
$125 for 3rd late cancel/no show in calendar year
$190 for 4th+ late cancel/no show in calendar year
Appointment reminders are sent as a courtesy. It is your responsibility to know when you have an appointment scheduled. Not getting an appointment reminder or not seeing it until it was within 48 hours does not waive late cancellation fees.
Already see a therapist in our practice and want to book an appointment with the nurse practitioner?
Call us at 614.360.2600. We will share your chart with Ms. Davis and schedule your appointment.
Please do NOT register as a new patient for Ms. Davis if you are a current patient in the practice.
Need to communicate with your provider?
Send a secure message through the PATIENT PORTAL or go to their profile page and use the form at the bottom to send them a message.
Have questions about how to do the telehealth appointment?
Make sure you sign the informed consent for telehealth that your provider assigns to you in the patient portal.
You only need to complete the form once. It is not specific to provider and does not need to be completed for every appointment.
We need one signed consent for each patient, including couples.
