Schedule online in 3 easy steps!
Step 1: Choose a provider
**Not all providers are accepting new patients. 
Counselors/Psychologists provide therapy services only. They cannot prescribe medications.
If you need medication services, you will need to schedule with a prescriber. If you need BOTH counseling and medication services,
​register for counseling and then call us at 614.360.2600, option 1, to add a prescriber to your profile.

Step 2: Register and complete paperwork
  1. Click the "Schedule with (provider's name)" button at the top of the provider's profile page. You will be taken to Therapy Appointment.
  2. Click "Book an Appointment" under new patient.

Please allow plenty of time to complete the registration process so you are not rushed or unable to finish. Follow the prompts to complete each tab in the registration process. Click CONTINUE at the bottom of each screen to advance to the next section.
Complete:
  • Demographics (name, address, phone number, email, etc)
  • Identify verification
  • Insurance information, if applicable
  • HSA or credit card information (used for any applicable copays/fees)
  • Consents/new patient forms*

*CONSENTS/NEW PATIENT FORMS
If you SKIP the paperwork during the registration process, you MUST log in to the patient portal and complete the paperwork at least 2 days prior to your scheduled appointment. (Under Docs&Forms in your portal.)
Step 3: Activate the patient portal and finalize appointment
1. Confirm appointment

2. Activate your patient portal (REQUIRED)
  1. Click "Set up Username and Password" button on the final screen
  2. Enter your birthday and create a Username and Password. 
  3.  You have now activated your patient portal, which is necessary for telehealth.​
​
​REQUIRED PAPERWORK (SIGNED CONSENT FORMS)
*If you skipped the consent forms/paperwork during the registration process, you MUST log in to the patient portal and complete the paperwork at least 2 days prior to your scheduled appointment. (Under Docs&Forms in your portal.) If paperwork is not received at least 2 days prior to the appointment, the appointment will be cancelled and you will be invited to re-schedule after completing the paperwork.

​Started the registration process but got out before finishing? No problem!
If you completed the demographic screen and verified your identity (in step 2), check your email for a message from NoReply@TherapyAppointment.com. It has a link to activate your patient portal. You will be able to pick up where you left off. Call us at 614.360.2600, option 1, if you need help at any time.
PARENTS: If your child is 18 years or older we are NOT permitted to talk to you, even to schedule/reschedule appointments, unless there is a signed Release of information form (ROI) on file.
