Step 3: Activate the patient portal and finalize appointment

If you completed the demographic screen and verified your identity (in step 2), check your email for a message from NoReply@TherapyAppointment.com. Click on the link to activate your patient portal. Call us at 614.360.2600, option 1, if you need help at any time.

PARENTS: If your child is 18 years or older we are NOT permitted to talk to you, even to schedule/reschedule appointments, unless there is a signed Release of information form (ROI) on file.
